Type 1 diabetes can be treated successfully by islet allotransplantation, the results of which are steadily improving. However, the number of islets that can be obtained from deceased human donors will never be sufficient to cure more than a very (...) small percentage of patients who might benefit from transplantation. Although there are some differences in glucose metabolism between pigs and humans, the use of pigs could provide an unlimited supply of islets, and the insulin produced would undoubtedly control glucose levels. Transplantation of islets into the portal vein results in islets residing in the liver; however, an early inflammatory response and rejection remain problematic, even when the recipient is receiving immunosuppressive therapy. In the long term, immunosuppressive drugs may exhibit toxicities to patients and specifically harm the islet cells. In contrast, encapsulation techniques provide islets with a physical barrier that prevents antibodies binding to the islet graft while still allowing insulin to be released into the recipient's circulation; in theory, patients receiving encapsulated grafts might not require exogenous immunosuppressive therapy. Nonhuman primates with encapsulated pig islet transplants have remained insulin-independent for several weeks, but long-term efficacy remains uncertain. Furthermore, techniques are now available to knock out genes from the pig and/or insert human genes, thus rendering the antigenic structure of pigs closer to that of humans, and providing protection from the human immune response. Islet transplantation from genetically engineered pigs has been followed by insulin independence in a small number of nonhuman primates for greater than 1 year. Neonatal islets have some advantages over adult islets in that they are easier to isolate and culture, and have the ability to proliferate during the first few months after transplantation. In 2009, the International Xenotransplantation Association set up a group to encourage and advise on clinical trials of pig islet xenotransplantation; this group's guidelines are discussed. Background: Issues concerning legislation and regulation with respect to the role of nurses in euthanasia and physician-assisted suicide gave the Minister for Health reason to commission a study of the role of nurses in medical end-of-life decisions in hospitals, home care and nursing homes.Aim: This paper reports the findings of a study of the role of nurses in euthanasia and physician-assisted suicide, conducted as part of a study of the role of nurses in medical end-of-life decisions. The findings for hospitals, (...) home care and nursing homes are described and compared.Method: A questionnaire was sent to 1509 nurses, employed in 73 hospitals, 55 home care organisations and 63 nursing homes. 1179 responses were suitable for analysis. The questionnaire was pilot-tested among 106 nurses, with a response rate of 85%.Results: In 37.0% of cases, the nurse was the first person with whom patients discussed their request for euthanasia or physician-assisted suicide. Consultation between physicians and nurses during the decision-making process took place quite often in hospitals and nursing homes and less frequently in home care situations . In some cases , nurses administered the euthanatics.Conclusions: The results show substantial differences between the intramural sector and the extramural sector , which are probably linked to the organisational structure of the institutions. Consultation between physicians and nurses during the decision-making process needs improvement, particularly in home care. Some nurses had administered euthanatics, although this task is by law exclusively reserved to physicians. (shrink)

This paper reports a study of planning processes in the five-disc Tower of London (TOL) task in 20 younger and 20 older adult participants. A concurrent direct ''think-aloud'' method was used to obtain data on planning processes prior to moving discs in the TOL. A check was made of the effects of verbalising by comparing performance data from the experimental groups with data from control groups who did not verbalise during planning or moving. Verbalising slowed down planning and moving but (...) did not appear to distort the participants' approaches to the task. Older and younger participants did not differ in average moves taken to solve the tasks. However, older participants' planning was less complete and more error-prone than that of younger participants. The planning processes were characterised as showing a means-ends ''goal selection'' strategy. In this strategy participants (1) identify a single active goal disc at the start, (2) select moves and move sequences to enable the placing of the current goal disc in its target position, and (3) continue in this way until all discs are in their target positions. Age differences were found in the planning stage, during which there was no stimulus support and hence a substantial working memory load. During the move phase there was stimulus support and hence little loading of working memory. Age differences in moves required were not found in the move phase. As older participants tend to have depleted working memory capacity the present results suggest that working memory is heavily loaded in TOL planning but less so in the move phase of TOL. (shrink)

This paper reports a study of the roles of visuo-spatial and verbal working memory capacities in solving a planning task - the five-disc Tower of London (TOL) task. An individual differences approach was taken. Sixty adult participants were tested on 20 TOL tasks of varying difficulty. Total moves over the 20 TOL tasks was taken as a measure of performance. Participants were also assessed on measures of fluid intelligence (Raven's matrices), verbal short-term storage (Digit span), verbal working memory span (Silly (...) Sentence span), visuo-spatial short-term storage (Visual Pattern span and Corsi Block span), visuo-spatial working memory (Corsi Distance Estimation), visuo-spatial processing speed (Manikin test), and verbal speed (Rehearsal speed). Exploratory factor analysis using an oblique rotation method revealed three factors which were interpreted as (1) a visuo-spatial working memory factor, (2) an age-speed factor, and (3) a verbal working memory factor. The visuo-spatial and verbal factors were only moderately correlated. Performance on the TOL task loaded on the visuo-spatial factor but did not load on the other factors. It is concluded that the predominant goal-selection strategy adopted in solving the TOL relies on visuo-spatial working memory capacity and particularly involves the active ''inner scribe'' spatial rehearsal mechanism. These correlational analyses confirm and extend results previously obtained by use of dual task methods, (Phillips, Wynn, Gilhooly, Della Sala, & Logie, 1999). (shrink)

According to the Einstein-Mayer theory of the Riemanniann space-time with Einstein-Cartan teleparallelism, the local Lorentz invariance is broken by the gravitational field defining Machian reference systems. This breaking of symmetry implies the occurrence of “hidden matter” in the Einstein equations of gravity. The hidden matter is described by the non-Lorentz-invariant energy-momentum tensor $\hat \Theta _{ik}$ satisfying the relation $\hat \Theta _{i;k}^k = 0$ . The tensor $\hat \Theta _{ik}$ is formed from the Einstein-Cartan torsion field given by the anholonomy objects, (...) FAik=2hA[i,k], and appears together with Hilbert’s energy-momentum tensor T* ik and Poincaré’s pressure λgik on the right-hand side of Einstein’s equations so that one has $$R_{ik} - (1/2)g_{ik} R = - \kappa T*_{ik} - \lambda g_{ik} - \hat \Theta _{ik}$$ According to this theory, in the universe and in cosmic systems one must excep “invisible masses” described by the Poincaré and Einstein-Cartan terms to exist. The torsion field FAik makes the space-time a Machian universe; it is of the same nature as the “weak interacting matter” discussed in astrophysics. (shrink)

Empirical and theoretical evidence show that the astrophysical problem of dark matter might be solved by a theory of Einstein-Mayer type. In this theory, up to global Lorentz rotations, the reference system is determined by the motion of cosmic matter. Thus, one is led to a “Riemannian space with teleparallelism” realizing a geometric version of the Mach-Einstein doctrine. The field equations of this gravitational theory contain hidden matter terms, where the existence of hidden matter is inferred solely from its gravitational (...) effects. It is argued that, in the nonrelativistic mechanical approximation, they provide an inertia-free mechanics, where the inertial mass of a body is induced by the gravitational action of the comic masses. Interpreted from the Newtonian point of view, this mechanics shows that the effective gravitational mass of astrophysical objects depends on r such that one expects the existence of dark matter. On April 26, 1995, the United States Supreme Court limited the reach of the preemption provision of ERISA in New York State Conference of Blue Cross & Blue Shield Plans v. Tavelers Insurance Co. ). In Travelers, the Supreme Court upheld the validity of a New York statute requiring hospitals to collect surcharges from patients covered by commercial insurers and requiring health maintenance organizations to pay a surcharge to the state's general fund that varies depending on the number of Medicaid-eligible (...) HMO members enrolled. Neither patients insured by Blue Cross/Blue Shield plans nor the plans themselves are similarly charged under the statute. Several commercial insurers and HMOs claimed the surcharges should be preempted under ERISA because they are levied on patients whose insurance or HMO membership premiums are purchased by an ERISA plan.The ERISA preemption clause means that federal regulation, or lack thereof, supersedes state regulation that is related to employee pension and benefit plans. This preemption clause prevents states from changing health care and benefit law for a significant portion of state residents. (shrink)

© 2015 Elsevier Inc.Postnatal organogenesis occurs in an immune competent environment and is tightly controlled by interplay between positive and negative regulators. Innate immune cells have beneficial roles in postnatal tissue remodeling, but roles for the adaptive immune system are currently unexplored. Here we show that adaptive immune responses participate in the normal postnatal development of a non-lymphoid epithelial tissue. Since the mammary gland is the only organ developing predominantly after birth, we utilized it as a powerful system to study (...) adaptive immune regulation of organogenesis. We found that antigen-mediated interactions between mammary antigen-presenting cells and interferon-γ -producing CD4+ T helper 1 cells participate in MG postnatal organogenesis as negative regulators, locally orchestrating epithelial rearrangement. IFNγ then affects luminal lineage differentiation. This function of adaptive immune responses, regulating normal development, changes the paradigm for studying players of postnatal organogenesis and provides insights into immune surveillance and cancer transformation. To localize wheat ESTs on chromosomes, 882 homoeologous group 6-specific ESTs were identified by physically mapping 7965 singletons from 37 cDNA libraries on 146 chromosome, arm, and sub-arm aneuploid and deletion stocks. The 882 ESTs were physically mapped to 25 regions flanked by 23 deletion breakpoints. Of the 5154 restriction fragments detected by 882 ESTs, 2043 were localized to group 6 chromosomes and 806 were mapped on other chromosome groups. The number of loci mapped was greatest on chromosome 6B and (...) least on 6D. The 264 ESTs that detected orthologous loci on all three homoeologs using one restriction enzyme were used to construct a consensus physical map. The physical distribution of ESTs was uneven on chromosomes with a tendency toward higher densities in the distal halves of chromosome arms. About 43% of the wheat group 6 ESTs identified rice homologs upon comparisons of genome sequences. Fifty-eight percent of these ESTs were present on rice chromosome 2 and the remaining were on other rice chromosomes. Even within the group 6 bins, rice chromosomal blocks identified by 1-6 wheat ESTs were homologous to up to 11 rice chromosomes. These rice-block contigs were used to resolve the order of wheat ESTs within each bin. (shrink)

This publication of the proceedings of the first of a new series of colloquia to be held at the University of Western Ontario contains an opening address on existence and quantification by W. V. Quine and three symposia. The paper discussed in the first of these symposia, "Descartes' Ontological Argument" by Anthony Kenny, follows closely one chapter of Kenny's recent book on Descartes. Kenny's paper contains both an interesting account of Descartes' views and some challenging remarks about ontological arguments in (...) general. His paper is discussed in separate commentaries by Norman Malcolm and Terence Penelhum. An interesting feature of this volume is that some comments from the floor are recorded and included along with the regular commentaries. Ernest Sosa and Bernard Williams comment on Kenny's paper along with Malcolm and Penelhum. Kenny responds to all four commentators. The other two symposia follow a similar format. The second, "On Events and Event-descriptions," features a paper by R. M. Martin which focuses on the logic of event descriptions and their role in scientific explanation. Events for Martin are extensional entities, unlike facts, but like facts they are "fictitious in the sense of being handled on virtually." His views are discussed by Donald Davidson, R. J. Butler and Wesley Salmon. In the third symposium on "Existence Assumptions in Practical Thinking," Stephan Körner elaborates on his view of effective choice, relating his account to those of Hume, Kant, and Peirce on the specific topic of the relationship of choice and natural necessity. The commentaries of J. J. Thomson and Bernard Williams are helpful here in getting Körner to clarify his interesting views. In sum, this first colloquium at the University of Western Ontario features genuine philosophical communication on some important issues.--R. H. K. (shrink)

When in 1950, the distinguished psychologist, Jean Piaget, published a book on the relation of logic and psychology, the book was severely criticized in the journal Methodos by the logician E. V. Beth. Piaget asked to get together with Beth to discuss the issues involved. The result, over 15 years later, is the present book. Beth is the author of the first half in which he defends the complete autonomy of logic in relation to psychology by means of a partly (...) philosophical, partly historical treatment of logicist, formalist, and intuitionist philosophies of mathematics. Briefly put, Beth's thesis is the now familiar one in contemporary philosophy that logic and mathematics are normative sciences, psychology is a factual science. As a consequence, Beth argues that we must renounce all "psychologism" in logic and mathematics, and all "logicism" in psychology. Piaget has come to accept these main contentions of Beth and affirms them in the second half of the volume. But Piaget is impressed by the fact that a good deal of human conceptual development must go on before human beings recognize and state normative laws of logical thought. Piaget attempts to chart this conceptual development and explain how it takes place. He attempts to show that this genetic-psychological approach to the study of logical and mathematical thought is complementary to and fully compatible with the deductive approach of the formal logician, and hence compatible with Beth's thesis about the conceptual autonomy of logic and psychology. Logician and psychologist can thus deal with the same material although their approaches are conceptually autonomous. The book repays careful reading not only for the arguments defending these challenging claims but also for many other details in both parts.--R. H. K. (shrink)
